Mary beth bowns excellence in trauma care award

The purpose of the Mary Beth Bowns Excellence in Trauma Care Award is to reward an individual or organization in the State of Georgia for their outstanding delivery of care to a critically injured patient(s). Named in honor of Mary Beth Bowns, the recipient should exhibit excellence in the delivery of care to the trauma patient and / or effected positive change in the trauma care system. CRITERIA FOR CONSIDERATIONAlong with the online nomination form, please include a nomination statement that describes in narrative form why you believe the nominated individual (health professional, public official, volunteer) should receive the Mary Beth Bowns Excellence in Trauma Care Award. This should be no more than two (2) type written pages. Maximum of 2 recipients, individual, crew or service will be awarded. Consideration should be given to the following criteria:
DOCUMENTATIONSubmission of supporting documentation is encouraged. Examples would include EMS trip reports, commendations, public education, etc. All narrative information and documentation is to be uploaded to the nomination form. Information needed on nomination form:
